We are looking for the CMS de Payerne:

A social worker 50-100%

Mission:

• Ensure the entirety of social follow-up at the home of persons presenting a state of physical or mental dependency or a disability.
• Ensure the "situation reference" of clients and evaluate their needs.
• Actively and multidisciplinarily participate in the organization and follow-up of the care project defined with the client.

Profile:

• Holder of a Bachelor's in social work with an orientation in social service or a title deemed equivalent.
• Experience, if possible, in the social or medico-social field.
• Good knowledge of social insurance and the Vaudois medico-social network.
• Ability to work autonomously and in an interdisciplinary team.
• Ability to listen and negotiate.
• Holder of a driver's license and possess a vehicle (possibility to use a professional vehicle).
