We are looking for the CMS de Payerne: A social worker at 50-60% Mission: * Ensure all social follow-up at the homes of people presenting a state of physical or mental dependency or a disability. * Ensure the << situation reference >> of clients and assess their needs. * Actively participate in a multidisciplinary manner in the organization and follow-up of the care project defined with the client. Profile: * Holder of a Bachelor's in social work with an orientation in social service or a title deemed equivalent. * Experience, if possible, in the social or medico-social field. * Good knowledge of social insurance and the Vaud medico-social network. * Ability to work autonomously and in an interdisciplinary team. * Listening and negotiation skills. * Holder of a driver's license and possess a vehicle (possibility of using a professional vehicle).
