To support our teams, we are looking for, immediately or by arrangement: Nursing Specialist HF/FH 60-70% Location Kilchberg Your tasks Needs assessments according to interRAI-HC Independent, self-responsible care and support of customers Assumption of case management Determining nursing needs and nursing effort Creation, adaptation and evaluation of nursing documentation according to interRAI-HC Performing and documenting interventions according to nursing planning Conducting advisory consultations Continuous review of the quality and effectiveness of the care Collaboration with relatives, doctors, hospitals, homes and other specialist agencies Profile requirements a completed training (HF/FH) professional, independent way of working broad professional experience social competence, resilience flexibility, especially regarding working hours good PC skills driving license Cat.
