Registered Nurse HF / FH in psychiatry - or with experience in psychiatry 20 - 40%

Start date by agreement

Your area of responsibility:

• Psychiatric and psychosocial care and support of people with chronic and mental illnesses at home
• You perform needs assessments with interRAI-CMH and assume case management
• Autonomous implementation and planning of the global care process
• You work independently with clients, but are networked with prescribers, psychiatrists / psychologists and general practitioners.

What you bring:

• Diploma in nursing HF / FH with specialization in psychiatry or professional experience in the inpatient psychiatric field
• Autonomous, responsible and team-oriented way of working
• Reliability and flexibility
• Category B driver's license, personal car an advantage
