Rigorous follow-up: Supervise and regularly reassess situations, coordinate actions essential to safety and continuity of care. Document management: Responsible for managing OPAS files and updating client files in accordance with directives. Interdisciplinary collaboration: Work closely with the entire network (CMS team, caregivers, doctors, partners) to ensure optimal care. YOUR PROFILE You hold a HES nursing diploma or an equivalent title, reinforced by significant experience of 2 years, preferably in home care. You are versatile, attentive and autonomous; possess excellent interpersonal and negotiation skills and show a pronounced interest in home care. Your sense of organization, your ability to anticipate needs and adapt to any type of situation and your ability to manage stress are essential qualities.
