Mental Health Reference Nurse (50–70 %)
Located in Château-d’Œx, at 1000 meters altitude, the Pôle Santé Pays-d’Enhaut offers a wide range of hospital services, accommodation, elderly care, as well as home care. According to the so-called integrated care approach, this innovative organization represents an opportunity for health professionals to work within small multidisciplinary teams, for better continuity of care and in response to the health needs of the population of Pays-d’Enhaut.

For its Home Care Sector, the Pôle Santé du Pays-d’Enhaut is looking for a:
Mental Health Reference Nurse (50–70 %)
Start date: 1st December 2026
In principle attached to the home care sector, you will also be required to carry out occasional missions within the accommodation and hospital sectors.
Your mission:
Assess needs, develop and adapt care projects according to the evolution of the situation
Provide medico-technical nursing care and actively contribute to the team dynamics
Participate in B13 follow-ups in collaboration with the professionals concerned
Ensure appropriate care for people of all ages, in collaboration with their surroundings
Guarantee rigorous transmission and documentation of information necessary for the follow-up of situations
Promote staying at home and support the autonomy of beneficiaries
Participate, in a multidisciplinary way, in the organization and follow-up of the care project defined with the client
Your profile:
Bachelor HES-SO in nursing or a diploma recognized by the Swiss Red Cross
CAS in mental health or training deemed equivalent;
2 years of experience in the psychiatric field
Listening skills, autonomy and ability to manage complex situations
Proactivity and ease in network work
Proficiency in usual IT tools
Driving license and vehicle indispensable
Knowledge of home care: an asset
