Mandated by a CMS in the canton of Vaud, we are looking for

Job description ###

• Accompany patients in their life project at home, in collaboration with their relatives and the care network
• Coordinate global care with interdisciplinary teams and health partners
• Develop and adapt the therapeutic project according to the evolution of needs
• Perform evaluations, nursing care, and necessary basic care
• Promote staying at home, prevention, health promotion, and patient autonomy
• Participate in the continuous improvement of services and development projects

Profile ###

-Bachelor in nursing or a title deemed equivalent (foreign diploma recognized by the CRS)

• Experience in the medico-social field, with a marked interest in community health
• Ability to work autonomously and to make appropriate decisions
• Aptitude to collaborate within an interdisciplinary network and a multidisciplinary team
• Sense of responsibility, rigor, and respect for professional ethics
